Requirements to Participate
Important: For all activities that compete in MSHSAA sponsored competitions, including Lancer sports, cheer, poms and marching band
- Students must be enrolled in and successfully earn credit in at least 3.0 units of credit (6 classes) each SEMESTER. Incoming freshmen are automatically eligible for the fall semester. (Summer school classes MAY be used to earn up to 1.0 units of credit towards eligibility for first semester for certain classes. Check with the Activities Director to see if a class would qualify.)
- All students must adhere to the Rockwood Citizenship Policy (available within the link below-'Activities and Athletics Guideline Handbook'). This includes conduct both in and out of school, as well as in and out of season for their activity/sport. FinalForms is used to submit your MSHSAA-mandated physical and emergency information online. Two important details with this submission:
- Your physical you upload to FinalForms must be dated 7/1/2024 or later in order for it to be considered valid.
- You must complete the online FinalForms process before attending the first day of tryouts/practice in order to participate.
Note: According to MSHSAA By-Laws 3.8.1 and 4.5.6, schools shall require every student participating in spirit activities (sideline cheer and dance teams) and athletics to present a physician's certificate stating that he or she is physically able to participate. These By-Laws do not allow for any exceptions to this requirement.
Moving from PRIVIT to FinalForms
We are very excited to announce that the Rockwood School District has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. For those families who have a valid physical on file with PRIVIT, we recommend you download that physical prior to PRIVIT expiring in June for upload into the FinalForms platform.
We are asking the parents/guardians of all students planning to play sports to create accounts in FinalForms and register your student(s).
This system requires both a parent/guardian and your student to make an account. Both will need to log in and sign off on all documentation for full clearance. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update. If you require any support during the process, scroll to the page bottom and click “Get Help”.
- Please register at: https://rockwood-mo.finalforms.com/ and follow the prompts to create your account, create your students and sign your forms.
- All participants MUST use this MSHSAA form for their physical exam and ONLY UPLOAD Page 5 of this document to Final Forms digitally.
All Students will need to upload their MSHSAA Physical Exam into final forms this first year, it will NOT transfer from PRIVIT.
Registration Deadlines
Please note, your student(s) CANNOT participate until marked Eligible (highlighted in green) in FinalForms. The dates below are first dates of the seasons therefore you need to have this information submitted a MINIMUM of 1 week prior to the dates below.
